How Can I Gain Access to an iTASC Surgeon?

1. Ask your referring provider to register on the iTASC website.
2. Financially qualify yourself using the iTASC income threshold chart

The referring provider must verify the patient meets current Federal Income thresholds. The following information can be used as a general guideline:

3. Qualify your cancer based on Mohs AUC Criteria

Mohs micrographic surgery involves step-by-step removal of cancerous tissues in layers. Each layer is examined under a microscope in real time. It is effective for cancers in cosmetically or functionally sensitive areas, and for recurrent or aggressive tumors.

The patient must meet the Mohs AUC criteria of a 7,8 or 9 to qualify.

4. Ask your referring provider to fill out an iTASC one-step referral form

Ask your referring provider to fill out an iTASC one-step referral form and fax to 937-345-2351 with the following three documents.

Frequently Asked Questions

How does a patient with a suspicious lesion for skin cancer get referred to iTASC?

The patient needs to have a biopsy proven skin cancer to be referred to iTASC. A biopsy of a suspicious growth may be obtained at a free clinic, Federally Qualified Health Center, University clinic or Essential Hospital.

How are patients qualified?
  • Your referring physician’s office will verify whether the patient falls at or below 220% of the Federal Poverty Level based on household size before registering the patient on iTASC.
  • Only patients that have skin cancer that meet the Mohs AUC criteria with a 7, 8 or 9 score will be treated through iTASC.
How are Mohs surgeons identified and qualified?
  • Volunteer Mohs micrographic surgeons must be board certified and carry an active medical license. This will be verified by iTASC administration at the Federation of State Medical Boards (FSMB) website.
  • The micrographic surgeons must also be members of the American College of Mohs Surgery.
